An unannounced monitoring inspection was conducted on August 30, 2023. This on-site visit was conducted from 10:50 a.m. through 11:30 a.m. Upon arrival, there were 17 children, ages 4 through 45 in the care of 4 staff members. Five children and four staff records were reviewed.

Standard #: 22.1-289.035-A Description: Based on review of staff records the center failed to obtain an updated Virginia Central Registry finding for one staff member.
Evidence:
1. Four staff records were reviewed.
2. Staff #2 did not have documentation of obtaining an updated Virginia Central Registry Search within five years of the previous check.
3. Staff 2 Virginia Central Registry check was documented 12/20/2015.
4. Administration confirmed an updated Virginia Central Registry check had not been obtained within 5 years of the previous one for Staff #2.Plan of Correction: Not available online. Contact Inspector for more information.
