Comments:
A Subsidy Health and Safety inspection was conducted on August 8, 2023 to review supplemental health and safety requirements. This on-site visit was conducted from 11:25 am through 12:00 pm. Upon arrival, there were 61 children, ages from 4 through 11, with 6 staff supervising. Five children records and seven staff records were reviewed and updated.

Standard #: 8VAC20-790-560-A
Description: Based on record review, the facility failed to have one staff member obtain a tuberculosis screening prior to coming into contact with children.

Evidence;

1. Staff records review revealed Staff #6 did not have documentation of obtaining a tuberculosis screening prior to coming into contact with children. Date of hire was documented 08/11/2022.
2. The director confirmed a tuberculosis screening had not been obtained.

Plan of Correction: Staff will get TB test on September 29th. Staff #6 was hired on 6/12/2023 not 8/11/2022.

Standard #: 8VAC20-790-600-F
Description: Based on record review the center failed to have one staff member complete the departments annual update health and safety course.

Evidence:

1. Review of staff records revealed Staff #2had not taken the required annual 3-hour health and safety update course. Last documented course was dated 03/08/2021.
2. The director confirmed the annual 3-hour update had not been taken.

Plan of Correction: Staff will get TB test on September 29th. Staff #6 was hired on 6/12/2023 not 8/11/2022.
