Comments:
The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on Monday, September 18, 2023 from approximately 9:30 AM to 12:35 PM. There were 79 children in attendance and a total of four 12 supervising. A total of seven children and six staff records were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-790-540-B
Description: Based on review of seven children?s records, the vendor did not ensure two records contained the required information.
Evidence:
The records for Child #4 (first day of attendance 08/22/2023) and Child #7 (first day of attendance 08/22/2023) did not contain the name, address, and telephone number of at least one person designated by the parent to contact in case of an emergency and the parent cannot be reached; names of persons other than the custodial parent who are authorized to pick up the child; written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection for the provision of medical treatment on religious or other grounds, and a written statement that the vendor will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the vendor.

Plan of Correction: I will contact the parents and request the information to add to the records.

Standard #: 8VAC20-790-580-A
Description: Based on review of seven children?s record, the vendor did not obtain documentation of immunizations required by the State Board of Health before one child began attending the center.
Evidence:
The record for Child #4 (first day of attendance 08/22/2023) did not contain a copy of immunization.

Plan of Correction: I will follow up with the parents and request a copy of the immunization records again.

Standard #: 8VAC20-790-640-A
Description: Based on observation, the vendor did not store hazardous substances such as cleaning materials, insecticides, and pesticides were kept in a locked place using a safe locking method that prevented access by children.
Evidence:
Cleaning materials were stored on a shelf in the children?s bathroom located in the Pre-K classroom.

Plan of Correction: We will move the cleaning products to a locked area.

Standard #: 8VAC20-790-760-A-2
Description: Based on observation, the vendor did not ensure children?s hand were washed after toileting.
Evidence:
A staff member in the Two?s classroom was observed changing a child?s diaper. After the diaper change, the child did not wash their hands. The child began participating in activities in the classroom.

Plan of Correction: I will review the required steps with staff to ensure both staff and children wash their hands at appropriate times.

Standard #: 8VAC20-790-760-A-3
Description: Based on observation, the vendor did not ensure staff washed their hands with soap and running water before and after helping a child use the toilet or changing a diaper.
Evidence:
A staff member in the Two?s classroom was observed changing a child?s diaper. After the diaper change, the staff did not wash their hands. The staff continued to complete tasks in the classroom.

Plan of Correction: I will review the required steps with staff to ensure both staff and children wash their hands at appropriate times.
