Licensing Inspector conducted an unannounced Religiously Exempt Code Monitor Inspection on October 4, 2023. There were 9 children ages 2 1/2 to 4 years old present under the direct care of 2 staff. The Licensing Inspector reviewed 4 children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.049-A Description: Based on record review, the Center failed to obtain documentation of proof of identity and age and previous child day care and schools for each child enrolled.
EVIDENCE:
Record for Child #1 did not include documentation that the Center confirmed proof of age and identity (birth certificate).Plan of Correction: Center Director will request documentation of proof of age/identity from the child's parents.
Standard #: 22.1-289.058 Description: Based on inspection & discussion with the Center Director, the Center failed to have a carbon monoxide detector in each building that was built before 2015 and that houses a child day program serving preschool-age children.
EVIDENCE:
A carbon monoxide detector was unavailable.Plan of Correction: Center Director will purchase and install a carbon monoxide detector.
