The licensing inspectors conducted an unannounced Subsidy Health and Safety inspection on October 17, 2023 from approximately 9:45 AM to 11:30 AM. There were 31 children in attendance and a total of five staff supervising. A total of five children and five staff records were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-790-630-A Description: Based on observation, the vender did not ensure all areas and equipment of the center, inside and outside, were maintained in a clean, safe, and operable condition.
Evidence:
Several areas of exposed landscaping fabric had worked its way to the playground surface. The exposed fabric created a tripping hazard.Plan of Correction: We will remove the exposed fabric and add additional mulch to the areas.
