A non-mandated shsi monitoring inspection was conducted on October 20, 2023 from 11:51A.M.-12:30 P.M., in response to a serious injury report received in the Office of Child Health and Safety on October 12, 2023. The inspector reviewed compliance in the areas of special care and emergencies. A total of one child record and two staff records were reviewed, staff were interviewed and review of injury reports and documentation were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-790-720-B Description: Based on staff interview, the licensee did not ensure the parent was notified by the end of the day of any known minor injuries.
Evidence: Staff #2 confirmed the center staff did not report child #1's thumb injury to the parent by the end of the day. The center did provide first aid to child #1 after the incident happened.Plan of Correction: A memo was provided to the staff about the procedures of documentating injuries in a report. The staff will need to notify preschool and school age parents of injuries . Document will be filed and sent home to parents.
Standard #: 8VAC20-790-720-C Description: Based on staff interview, the licensee did not ensure the vendor maintain written record of children's serious and minor injuries in which entries are made the day of occurrence.
Evidence : Child #1 thumb was injured on 10/11/2023. The written record of child's serious injury was completed on 10/12/2023. Staff #2 confirmed the center did not complete written record on the day of occurence.Plan of Correction: If a School age child is injured and recieve a bandaid or icepack the staff will write a report. Children with special needs if injury happens the injury report will be completed immediatley. The document will be filed and sent home.
