A subsidy health and safety inspection was conducted on October 12, 2023 from approximately 11:00 am to 11:30 am. A follow up inspection was conducted on 10/26/23 from approximately 12:00 pm to 1:00 pm There were 5 children present, ranging in ages from 3 to 4 years, with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of 5 children's records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-790-560-A Description: Based on record review, observation, and interview, the vendor failed to ensure that all staff submit documentation of a negative TB screening at the time of employment.
Evidence: The record for staff #1, date of employment 10/02/2023, did not contain documentation of a negative TB screening. Staff #1 was observed working alone with children. The Director confirmed that staff #1 did not have a TB screening completed prior to employment.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-790-600-F Description: Based on record review, and interview, the vendor failed to ensure that all staff who work directly with children shall annually attend the department's health and safety update course.
Evidence: The record for staff #2, date of employment 07/19/2021, did not contain documentation of the department's annual health and safety update course.
