Comments:
The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on October 24, 2023 from approximately 11:30 AM to 1:30 PM. There were 26 children in attendance and a total of six staff supervising. A total of five children and five staff records were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-790-540-B
Description: Based on review of five children's records, the vendor did not ensure five children's records contained the required information.
Evidence:
1. The records for Child #1, Child #3, and Child #4 did not contain the address for the listed emergency contacts.
2. The director confirmed this information during the inspection.

Plan of Correction: I will request and add the missing information to the children's records today.

Standard #: 8VAC20-790-560-A
Description: Based on review of five staff records, the vendor did not ensure one staff member obtained a negative tuberculosis (TB) screening within the required timeframe.
Evidence:
The record for Staff #4 (Date of employment 07/10/2023) contained documentation of a TB screening dated 01/11/2023. Staff are required to obtain a negative TB screening completed within 30 days prior to beginning employment.

Plan of Correction: Going forward, I will ensure all staff obtain the TB screening within the required timeframe.

Standard #: 8VAC20-790-600-C
Description: Based on review of five staff records and interview, the vendor did not ensure orientation training for staff was completed on specific topics prior to the staff member working alone with children and within seven days of the date of employment.
Evidence:
1. The record for Staff #3 (Date of employment 10/23/2023) did not contain documentation of orientation.
2. Staff #3 was observed working alone with the children in the classroom.
3. The director stated all the requirements of orientation had not been completed as of 10/24/2023.

Plan of Correction: I will ensure the orientation is completed going forward prior to staff working alone with children.

Standard #: 8VAC20-790-760-A-2
Description: Based on observation, the vendor did not ensure children?s hands were washed with soap and running water after toileting and any contact with blood, feces, or urine.
Evidence:
Staff in the Toddler/Two Classroom was observed changing three children?s diapers. After each diaper change, children were sat on the ground, and they continued to play in the classroom. The children?s hands were not washed.

Plan of Correction: I will continue reviewing with the staff the importance of handwashing and when it is required to do it throughout the day.

Standard #: 8VAC20-790-760-A-3
Description: Based on observation, the vendor did not ensure staff washed their hands with soap and running water before and after helping a child use the toilet or changing a diaper.
Evidence:
Staff in the Toddler/Two Classroom was observed changing three children?s diapers. After each diaper change, the staff member did not wash her hands.

Plan of Correction: I will continue reviewing with the staff the importance of handwashing and when it is required to do it throughout the day.

Standard #: 8VAC20-790-760-B-3-c
Description: Based on the observation, the vendor did not ensure a nonabsorbent surface for diapering or changing was used.
Evidence:
The diaper changing pad was covered with cloth material creating a non-absorbent diapering surface.

Plan of Correction: I will remove the cloth cover from the diaper changing pad.
