Comments:
The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on October 10, 2023 from approximately 10:30 AM to 1:30 PM. There were 59 children in attendance and a total of 18 staff supervising. A total of six children and six staff records were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-790-560-A
Description: Based on review of six staff records, the vendor did not ensure three staff were evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB) within the required timeframe.
Evidence:
1. The record for Staff #1 (Date of Employment 11/21/2022) contained a TB screening dated 12/05/2022.
2. The record of Staff #3 (Date of Employment 12/12/2023) contained a TB screening dated 09/22/2023.
3. The record of Staff #4 (Date of Employment 09/05/2023) contained a TB screening dated 09/22/2023.
4. The documentation of the TB screening is required to be obtained within the last 30 calendar days of the date of employment and be signed by a physician, physician?s designee, or an official of the local health department.

Plan of Correction: Going forward we will ensure all staff members begin employment with TB screenings obtained within the required timeframe.
