Comments:

The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on October 23, 2023 from approximately 3:00 AM to 4:20 PM. There were 21 children in attendance and a total of two staff supervising. A total of five children and two staff records were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	8VAC20-790-560-B
Description:	Based on review of two staff records, the vendor did not ensure subsequent tuberculosis (TB) screenings were required at least every two years from the date of the initial screening for one staff member. Evidence: The record for Staff #1 (Date of Employment 05/19/2021) contained a TB screening dated 08/19/2021.
Plan of Correction:	I will have the staff member obtain an updated TB screening.
Standard #:	8VAC20-790-600-D-1
Description:	Based on review of two staff records and interview, the vendor did not ensure all staff who work directly with children have within 90 days of the date of employment, current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care. The training must include an in-person competency demonstration. Evidence: 1. The record for Staff #1 (Date of Employment 05/19/2021) did not contain documentation of CPR certification. 2. A member of management stated Staff #1 had not completed the training.
Plan of Correction:	We will enroll the staff member in the class so the staff member will have the required training.
Standard #:	8VAC20-790-600-D-2
Description:	Based on review of two staff records and interview, the vendor did not ensure all staff who work directly with children have within 90 days of the date of employment, current certification in first aid appropriate to the ages of children in care. Evidence: 1. The record for Staff #1 (Date of Employment 05/19/2021) did not contain documentation of current first aid certification. 2. A member of management stated Staff #1 had not completed the training.
Plan of Correction:	We will enroll the staff member in the class so the staff member will have the required training.
Standard #:	8VAC20-790-640-A
Description:	Based on observation and interview, the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevented access by children. Evidence: During the inspection, the licensing inspector observed an unlocked cleaning chemicals sitting on shelf in two separate classrooms.
Plan of Correction:	These items were relocated to a locked cabinet while the licensing inspector was onsite. It was reviewed with all staff members that the cleaning chemicals needed to be locked up when not being used.
