Comments:
A monitoring inspection was conducted on December 5, 2023 approximately 9:50 am to 10:40 am.
There were 40 children present, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition.

The children were observed in circle time for reading. A total of 4 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-790-850-C
Description: Based on a review of children's records, the center did not ensure that all children in the center are in compliance with the requirements regarding the immunization of children against certain diseases for three of five children.
Evidence:
1. Child #2 (date of enrollment October 2023) records did not have documentation of immunizations.

Plan of Correction: Immunization records were mailed on 12/06/2023, to the inspector.
