A code compliance inspection was initiated and concluded on 1/8/2024. There were 32 children present with 4 staff supervising. Children were observed during teacher led activities and skill assessment. A total of 4 staff records were reviewed. The inspection started at 10:05 am and concluded at 11:00 am.
Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.031-A-4 Description: Based on record review and interview with staff, the center did not ensure each person in a supervisory position must be certified by a practicing physician or physician assistant to be free from any disability that would prevent them from caring for children under their supervision.
Evidence: Staff #4 confirmed that the following staff did not have proper documentation by a practicing physician or physician assistant:
1. Staff #1 had no documentation
2. Staff #4's most recent documentation was 7/9/2022Plan of Correction: Review all staff files & move up any dates for a health check.
