An initial Subsidy Health and Safety inspection (SHSI) was initiated on Thursday, January 11, 2024, and concluded on January 12, 2024. On January 11, the inspector was on site from 11:40am to approximately 2:17pm. There were 19 children in attendance with seven staff directly supervising. During the inspection, the children and staff were observed participating in a variety of activities. Staff were observed having positive interactions with the children. All areas of the facility including classrooms, hallways, bathrooms, and common areas were inspected. Medications are not administered, and transportation is not provided at this time. Five children's records and seven staff records were reviewed. Additional documentation was submitted to the inspector on January 12, 2024.
A code compliance inspection was conducted on the same dates as the SHSI supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the `plan of correction? and `date to be corrected? for each violation cited on the violation notice and returned it to me within five business days from today. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard #: 8VAC20-790-560-A Description: Based on a review of seven staff records, the vendor did not ensure six staff were evaluated by a health professional and were issued a statement that the individuals were determined to be free of communicable tuberculosis (TB) within the required timeframe.
Evidence: 1) The records of Staff #1, employed on 12/04/23, and Staff #6, employed on 12/04/23, did not contain documentation of a negative TB screening. 2) The TB screening in the record of Staff #2, employed on 01/02/24, was completed on 08/18/23. 3) The TB screening in the record of Staff #3, employed on 12/04/23, was completed on 12/10/23. 4) The TB screening in the record of Staff #4, employed on 12/04/23, was completed on 12/14/23. 5) The TB screening in the record of Staff #7, employed on 12/04/23, was completed on 04/01/23.
Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.Plan of Correction: Not available online.
