An unannounced monitoring inspection was conducted on January 24, 2024 This on-site visit was conducted from 11:15 a.m. through 12:00 p.m. Eight children, ages 3-4, were in the care of two staff. Five children and Three staff records were reviewed.

Standard #: 22.1-289.035-A Description: Based on record review the facility failed to obtain repeat fingerprint and central registry checks for one staff.
Evidence:
1. Three staff files were reviewed.
2. Staff #3 did not have documentation of obtaining repeat fingerprint and central registry checks. Staff #3 last documented fingerprint check was dated 09/15/2024. Central Registry check was dated 07/10/2018.Plan of Correction: Not available online. Contact Inspector for more information.
