A annual Religious exempt inspection was completed on February 14, 2024 the inspection was conducted from 11:50 am - 3:05pm. There were 75 children present. During the inspection children were observed transitioning from nap time and extended care and eating snack. Nine children records were reviewed and eight staff records were reviewed.Outdoor area was inspected.
Information gathered during the inspection determined non-compliance(s) with applicable requirements and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.031-A-4 Description: Based on record review, the center did not ensure each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision.
Evidence:
Staff #6 ( hire date 10/30/2023) does not have a health report to ensure to be free from any disability which would prevent him from caring for children under supervision.Plan of Correction: Discussed with staff member will get scheduled.
Standard #: 22.1-289.035-A Description: Based on record review and staff interview, the center di dnot ensure background checks are completed every five years.
Evidence: Staff #8 most recent fingerprint results were completed 7/17/2018. The results are more than 5 years old.Plan of Correction: The employee will be scheduled within the next week.
Standard #: 22.1-289.035-B-4 Description: Based on record review and staff interview, the center did not ensure to obtain a copy of the results of a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence: Staff #9 hire date 4/06/2023. Staff #9 lived in Utah in the past five years. The center did not have a Utah Child Abuse and Neglect registry background check for the state of Utah.Plan of Correction: Utah Background check is on file . File will be attached.
Per conversation , the center is no longer employed at the center as of March 2024.
