Licensing Inspector conducted an unannounced Religiously Exempt Code Monitor Inspection on April 4, 2024. There were 43 children ages 3 months to 5 years old present under the direct care of 10 staff. The Licensing Inspector reviewed 7 children?s records and 8 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-4 Description: Based on record review, the Center failed to obtain a copy of the results of a criminal history record information check, a sex offender registry check and a search of the child abuse and neglect registry from any state in which a prospective staff person has resided in the preceding five years.
EVIDENCE:
