Comments:
A Subsidy Health and Safety Inspection was initiated on Thursday, March 21, 2024 and concluded on Friday, March 22, 2024. On March 21, the inspector was on site from 9:40am to approximately 11:45am. There were 26 children present with six staff members directly supervising. A total of five children's records and five staff records were reviewed. Additional documentation was submitted electronically on March 22, 2024.

A code compliance inspection was conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in the code compliance inspection were reviewed during the supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-790-540-B
Description: Based on a review of five children's records, the vendor did not ensure three children's records contained the required information.

Evidence: The following records did not contain the address of at least one person designated by the parent to contact in case of an emergency and the parent cannot be reached - Child #1 (date of attendance: 05/30/23); Child #2 (date of attendance: 09/21/23); and Child #3 (date of attendance 02/06/23).

Plan of Correction: The missing information will be obtained for each child.

Standard #: 8VAC20-790-560-A
Description: Based on a review of five staff records and interview, the vendor did not ensure one staff was evaluated by a health professional and was issued a statement that the individual is determined to be free of communicable tuberculosis (TB) within the required timeframe.

Evidence: The TB screening in the record of Staff #1, employed on 06/19/23, was completed on 03/08/23. During interview, a member of management confirmed that was the only TB screening on file for Staff #1.

The documentation shall have been completed within the last 30 calendar days of the date of employment.

Plan of Correction: In the future, TBs will be required to be obtained no more than 30 days prior to employment.

Standard #: 8VAC20-790-580-B
Description: Based on a review of five children's records and interview, the vendor did not obtain documentation of additional immunizations once every six months for one child less than two years of age.

Evidence: The most recent immunizations in the record of Child #3 (15 months old), date of attendance 02/06/23, were obtained on 02/01/23 and on 03/21/24. During interview, a member of management reported the updated immunizations were not obtained every six months as required.

Plan of Correction: Corrected.

Standard #: 8VAC20-790-640-A
Description: Based on observations, the vendor did not ensure hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence: In the Preschool classroom, the inspector observed cleaning materials on a shelf in the classroom. The cleaning materials were out of reach of the children, but they were not locked.

Plan of Correction: Corrected. The cleaning supplies will be maintained in a locked place when they are not in use.
