Technical Assistance:
Discussed requirement to document previous day care and schools atteneded by the child and background check time frames for Virginia and Out of state background check.

Comments:
A monitoring inspection was initiated on 04/09/2024 from 9:07am -12:59pm and concluded on 4/10/2024. There were approximatley 40 children present during the inspection. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies. A total of 6 staff records and 6 children records were reviewed. The children were transitioning from activiity rooms and bathroom.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.031-A-3-a
Description: Based on observation and staff interview, the licensee did not ensure one staff member to four children from ages zero to 16 months.

Evidence:

1- During the inspection on April 9, 2024 there were 2 staff with 11 children from approximatley 9:07am - 9:30am in the infant/toddler room. The ages ranged from infant to toddler age children.
2- The ratio for infant is 1 staff to 4 children from ages zero to 16 months.
3- Confirmed by staff there was only 2 staff and 11 children.

Plan of Correction: I will review the procedure of moving children from classroom to classroom when one classroom exceeds ratio with the floor supervisor and lead teachers. I will also schedule additional staff members whenever possible so that in the event that someone is late or calls out we would still be in ratio. I will also reiterate the importance of supervising the children at all times.

Standard #: 22.1-289.031-A-3-c
Description: Based on observation and staff interview, the center did not ensure there were one staff to eight children from ages 24 months to 36 months.

Evidence:
1- On April 9, 2024 all classes ages 2- 5 years were combined in the one room.
2- When classes are combined the ratio should be determined by the youngest child.
3- Upon arrival there was 1 staff member with 29 children in the large room.
4- The youngest child was two years of age.
5- The required ratio for 2 year old is 1 staff to 8 children.

Plan of Correction: I will review the procedure of moving children from classroom to classroom when one classroom exceeds ratio with the floor supervisor and lead teachers. I will also schedule additional staff members whenever possible so that in the event that someone is late or calls out we would still be in ratio. I will also reiterate the importance of supervising the children at all times.

Description: Based on record review, the center did not ensure one staff member to 10 children from the ages 36 months to five years.

Evidence:
1-Staff # 6 was alone with 12 children in the Preschool classroom. Confirmed with staff.
2- The ratio for children 36 months to five years is 1 staff to 10 children.

Plan of Correction: I will review the procedure of moving children from classroom to classroom when one classroom exceeds ratio with the floor supervisor and lead teachers. I will also schedule additional staff members whenever possible so that in the event that someone is late or calls out we would still be in ratio. I will also reiterate the importance of supervising the children at all times.
