Technical Assistance:
Technical Assistance was provided to the Director regarding out-of-state background check requirements.

Comments:
An unannounced Code Compliance inspection was conducted with the Director on 4/24/24 from 9:10am to 10:50am. There were 29 children in care, ranging in age from 18-months to six-years-old, supervised by five staff. Children were observed playing in structured learning centers. The required postings and classrooms were observed. Five child records and six staff records were reviewed for required documentation. The program met the requirement for staff on-site with current CPR/First Aid training as well as having a staff trained in Daily Health Observation. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 5/1/24. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 22.1-289.031-A-4
Description: Based on review of six staff records, the program did not ensure that each person in a supervisory position had been certified annually by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision. Evidence: The records for Staff #5 and Staff #6 did not contain documentation of an annual health report.

Plan of Correction: Ensure all staff obtain form before start. Staff missing forms are completing now.

Standard #: 22.1-289.031-B-5
Description: Based on observation, the program did not ensure that all areas were free of obvious injury hazards. Evidence: On 4/24/24 in the Toddler classroom for children ages 18-months and older, an electrical outlet was observed uncovered next to a low bookshelf that was within reach of children.

Plan of Correction: Ensure all rooms are free of safety hazards with daily walk throughs.

Standard #: 22.1-289.035-B-4
Description: Based on review of six staff records, the program did not obtain documentation of the results of a criminal history record check from each state in which staff members had resided within the past five years prior to employment, or the results of a search of the central registry from each state in which staff members had resided within the past five years within 30 days of employment. Evidence: 1. The record for Staff #1 contained documentation that the staff member had resided in Washington State within the past five years. There was no documentation of a criminal history record check or central registry check from Washington State on record for Staff #1. 2. The record for Staff #6 contained documentation that the staff member had resided in Arizona and Tennessee within the past five years. There was no documentation of a criminal history record check or central registry check from Arizona, or a central registry check from Tennessee on record for Staff #6.

Plan of Correction: Cross check sworn statements with list of participating states for background checks and fingerprints. Obtain additional checks as needed.

Standard #: 22.1-289.049-A
Description: Based on review of five child records, the program did not obtain documentation of proof of a child?s identity and age upon enrollment. Evidence: The record for Child E did not contain documentation of proof of the child?s identity and age.

Plan of Correction: Have missing child's birth certificate now and will ensure all future children present proof of birth.
