Comments:
An unannounced renewal inspection was conducted today from 12:00pm-1:25pm. There were 8 children (20 points) directly supervised by the provider and 2 staff. The home, 3 staff records, 5 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were finishing lunch and transitioning to nap. Diapering procedures were also observed. There was an abundant supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-111-240-A
Description: Based on observation, not all areas of family day home were maintained in a safe and operable manner. Evidence: In the downstairs play room the science observation table has a crack in the top of the plastic creating a sharp edge.

Plan of Correction: I will throw the table away.

Standard #: 22VAC40-111-520-C
Description: Based on observation, two of the six cribs in use did not have mattresses that fit snugly in the crib. Evidence: Two of the cribs have mattresses that fit inside the crib frame with a space that more than two fingers can be inserted between the mattress and the crib.

Plan of Correction: I will order new mattresses.
