An unannounced monitoring inspection was conducted on 5/31/19 from 9:55am until 10:50am. At the time of entrance, there were 4 children in care, for at total of 7 points, with the provider. A sample of 4 children's records and 3 provider/household member/assistant records were reviewed. Children were observed engaging in free play with a variety of age appropriate toys. The provider stated that medication is not currently being administered to children in care. The emergency preparedness plan, first aid and emergency supplies, documentation of emergency practice drills and family day home postings were reviewed. One violation in the area of administration is identified on the violation notice and was discussed with the provider during the exit interview.

Standard #: 22VAC40-111-80-A Description: Based on record review and interview, it was determined the provider did not obtain from the parent verification of the identity and age of the child within seven business days of the child's first day of attendance at the family day home. Evidence: 1. The record for child 1 does not contain documentation that the provider obtained verification of the identity and age of the child. Child 1 has a date of enrollment of 12/2/18 and was in care on the day of the inspection. 2. The provider confirmed that verification of the identity and age of child 1 was not documented in the child's record. Plan of Correction: The provider stated she will contact the parent and have then bring a copy of the child's birth certificate at pick-up today.
