A Subsidy Health and Safety inspection was conducted on April 30, 2024 to review supplemental health and safety requirements. This on-site visit was conducted from 9:50 a.m. through 10:35.a.m. Upon arrival, there were 17 children, ages from 3 through 5 with 3 staff supervising. Five children records and five staff records were reviewed.
Areas of non-compliance may be found on the violation notice. Please complete your plan of correction and return to the licensing inspector within 5 business days.
A licensing code compliance inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.

Standard #: 22.1-289.035-B-4 Description: Based on record review the facility failed to obtain an out of state criminal history and sex offender check for one staff member that resided out of the state of Virginia within the last five years.
Evidence:
1. Five staff records were reviewed.
2. Staff #1 did not have documentation of obtaining an out of state criminal history and sex offender check prior to the first day of employment. Date of hire 11/22/2023.
3. The director confirmed out of state checks had not been obtained for Staff #1Plan of Correction: Not available online. Contact Inspector for more information.
