Completing required annual training.

Comments:
An unannounced monitoring inspection was conducted today form 9:30am to 11:30 am with 6 children (21points) present with 3 caregivers. The physical plant was inspected. Children's/staff/household members records, emergency supplies, the playground, cribs and emergency plans were reviewed. The children were observed during free play, on the playground, hand washing and eating lunch. Lunch consisted of Fried rice with chicken and mixed vegetables, fruit and milk. The family day home had an abundance of age appropriate toys and books for the children. Areas of non compliance were cited on the Violation Notice. Standard #: 22VAC40-111-60-A
Description: Based on a review of 5 of 7 children's records, the provider failed to maintain up to date records at the family day home for each enrolled child. Evidence: 1) Child #1's file did not have documentation of addresses for the two emergency contacts or documentation of medication policy or the emergency plan. 2) Child #2's file did not have documentation of an address for mother's place of employment, a medical insurance policy number, the medication policy or the emergency plan. 3) Child #3's file did not have documentation of an address for father's place of employment, no documentation of information for emergency contacts or the emergency plan. 4) Child #4's file did not have documentation of liability insurance, the medication policy or the emergency plan. 5) Child #5's file did not have documentation of the emergency plan.

Plan of Correction: I will speak with parents and collect information and put in children's files.
