Comments:
An unannounced monitoring inspection was conducted today between 12:45pm and 2:15pm, there were 8 children (24 points) present with three caregivers. The physical plant, children's records, household records, staff records, menu's, emergency plans, attendance and emergency supplies were inspected. There were no medications on site today. The children were observed during floor play, interacting with teachers and napping. Areas of non compliance were cited in the Violation Notice. Standard #: 22VAC40-111-60-A
Description: Based on a review of 5 of the 8 children's records, the provider failed to maintain up to date records at the family day home for each enrolled child. Evidence: 1) Child #1's file did not contain documentation of medical insurance policy number. or documentation of parental authorization of emergency medical care. 2) Child #2's file did not contain documentation of a doctor's address or phone number, documentation of the first emergency contacts phone number, documentation of medical insurance policy number or documentation of the medication policy. 3) Child #5's file did not contain documentation of authorization for emergency pick up.

Plan of Correction: I will get information from parents and put in file.

Standard #: 22VAC40-111-280
Description: Based on observation, the provider failed to ensure that potentially poisonous substances will be stored in areas not accessible to children. Evidence: 1) Hand sanitizer was stored on the changing table. 2) Air fresher was stored on changing table.

Plan of Correction: Removed during inspection.
