An unannounced monitoring inspection was completed on July 25, 2019 from approximately 10:30a.m. to 12:00p.m. to review compliance of specific standards. Three children were in care, for a total of six points, with two caregivers. Caregivers and children were observed in the indoor space engaged in activities to include breakfast and circle time to include a reading game. Interviews and interactions were conducted and no medications are given. Five children?s records were reviewed. The Provider was available for the inspection and was present at the exit interview at which time inspection findings were reviewed. Standard #: 22VAC40-111-100-A Description: Based on a review of children?s records, the licensee failed to obtain documentation of a physical examination prior to the child?s attendance or within 30 days after the first day of attendance for two of five children. Evidence: 1. Child #2 and Child #3's records, enrolled on June 10, 2019, lacked documentation of physicals. 2. Caregiver #1 acknowledged documentation of these physicals was not obtained prior to attendance or within 30 days after the first day of attendance. Plan of Correction: The provider will obtain documentation of the physicals from the children's parent.
