Reviewed renewal application with the provider and will keep the capacity for 9 children.

An unannounced renewal study was conducted today from 10:45-12:30. One child was present for two points with one caregiver. Reviewed physical space, records two for children, all household members TB and background checks, trainings and nutrition. At this time the backyard is off limits to day care children. The provider and assistant have current first aid and CPR certification as well as the required hours for annual requirements. Observed the child in free play with toys and equipment. Lunch to be served is chicken and rice, milk and applesauce. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-60-B Description: Based on record review, 2 of 2 children's records are incomplete. Evidence: Child #1 is missing an annual update (last completed 10/30/2018) as well as a field trip form to walk to the local park. Child #2 is missing the field trip form to walk to the local park. Plan of Correction: The provider will request parent review the forms and complete the field trip form.
Standard #: 22VAC40-111-90-C Description: Based on record review, immunization updates for a child under two years of age for a child have not been obtained. Evidence: Last update for child #1 was 10/30/2018 Plan of Correction: The provider has asked the parent for the updated form at her two year checkup.
Standard #: 22VAC40-111-280 Description: Based on observation, a cabinet containing hazardous substances was unlocked. Evidence: The bathroom cabinet contained mouthwash and did not have a safety lock installed. Plan of Correction: All mouthwash will be transferred to the cabinet with the lock.
Standard #: 22VAC40-111-320-B Description: Based on observation, two electrical outlets were missing protective covers. Evidence: The two outlets in the bathroom did not have protective covers installed to protect children under 5 years of age. Plan of Correction: Replaced outlet covers during the inspection.
