Comments:

An unannounced monitoring inspection was conducted today, September , 2019 to determine the provider's compliance with family day home licensing standards. The inspection was initiated at 2:00 pm and concluded at 3:30 pm. Upon the inspector's arrival, eight children were in care with two caregivers for a total of 20 points. Some children were napping while two children were finishing lunch. Napping children were monitored. As children got up, diapering, bathroom breaks and hand washing was conducted. Children played with toys and read books with the provider. Later three additional children arrived from school for a total of 23 points. Children played domino races and on the small indoor slide. The provider and assistant interacted and participated in play with the children in a positive manner. Children provide their own meals. All areas of the home were inspected to include the outside play area and gates to the pond. All gates were locked. The Inspector interacted with the caregivers and children in care. The provider had all required information posted. The first aid kit and non-medical emergency supplies were checked and found complete. An evacuation drill was documented on 09/13/2109 and the the two required shelter-in-place drills for 2019 were completed on 04/16/2019 and 09/05/2019. Eight children's records and the two caregivers records were reviewed. Per the provider: Medication is not being administered, no transportation provided. The caregivers have current CPR & First aid certification. Violations cited during the licensing period were checked for corrections. There were no repeat violations. Description:	Based on review of eight children's records, the provider did not obtain the parents written acknowledgement of the receipt of the parent information. Evidence: 1. The record for child #8, enrolled 05/20/2019, did not contain the parents written acknowledgement of the parent information. 2. The provider acknowledge the a signed copy of the parent information was not in the record.
Plan of Correction:	Per the provider: A copy was given to the parent however she did not return a signed copy to me. I have contacted her and will have her sign a copy at pick up today.
