Comments:
An unannounced renewal inspection was completed on November 20, 2019 from approximately 11:45 a.m. to 1:00 p.m. Eight children, ages seventeen months to three years old, were in care, for a total of eighteen points, with two caregivers. Caregivers and children were observed in the indoor space engaged in activities to include free play, lunch, and nap time. Interviews and interactions were conducted and no medications are given. Five children?s records, two caregiver records and three pet records were reviewed. The provider was available for the inspection and was present at the exit interview at which time inspection findings were reviewed. Standard #: 22VAC40-111-60-B
Description: Based on a review of children?s records, the licensee failed to maintain all the elements of a child?s record for four of five children.
Evidence: 1. Child #1's record lacked emergency contact addresses and Child #2, Child #3, and Child #5's records lacked an insurance policy number. 2. Caregiver #1 acknowledged this information was not documented in these records.

Plan of Correction: The provider will obtain this information from the parents.
