Comments:
An unannounced monitoring inspection was conducted today, December 10, 2019 to determine the provider's compliance with family day home standards. The inspector was on site from 11:40 pm until 1:20 pm. The inspector met with the provider and Family Day Home standards were reviewed. Ten family day home children were present with three caregivers for a total of 32 points. Upon the inspectors arrival, children were on the fun bus for a gymnastic class. Upon returning to the house, children had bathroom breaks, washed their hands and prepared for lunch. Today's lunch consisted of mac and cheese, broccoli, applesauce and milk. Children also watched a Christmas video and sand Christmas songs for the inspector. napping. Interactions between children and the caregivers were positive. All areas of the family day home were inspected. The family day home is equipped with age appropriate toys and supplies. The provider had the following information posted; license, last inspection notice, emergency numbers and the menu. The first aid kit and non-medical emergency supplies were inspected and found complete. The inspector interacted with caregivers and children in care.

Ten children's records and three caregiver records were reviewed along with two household member records.
Medication was on site and reviewed. One caregiver has MAT training. Two caregivers have current CPR and first aid.
The most recent evacuation drill was dated 11/25/2019. The two required shelter-in-place drills for 2019 were completed on 09/30/2019 and 11/25/2019. The emergency preparedness is current.

Standard #: 22VAC40-111-560-A
Description: Based on observation and interview, the provider did not maintain direct care and supervision of each child at all times.

Evidence:

1. Ten children were observed on the fun bus that was parked at the end of the driveway with two fun bus staff.
2. The provider and two caregivers were in the family day home out of direct care and supervision of the children.
3. The provider stated the fun bus staff told her they the caregivers did not need to be on the bus.

Plan of Correction: Per the provider: I didn't realize the staff had to be on the bus. In the past we have always been on the bus with the children however the fun bus owner said we did not need to be on the bus with the children. So we walked the children to the bus and go back to walk them to the family day home after the class.
