Technical assistance was offered related to children's records.

An unannounced renewal inspection was conducted from 10:25 am to 11:45 am. There were 10 children (28 points) in direct care and supervision of 2 caregivers. The children were observed playing with home living toys, dress up and doing circle time. Children were also observed washing hand after using the bathroom. There were no medications on site. 5 children's records, 2 caregiver records, and emergency drill records were reviewed. A walk through of the family day home was conducted. Standard #: 22VAC40-111-960-E Description: Based on observation, infant bottles were not labeled with the name and date.
Evidence: Three infant bottles for Child # 1 were not dated.Plan of Correction: Bottles will be labeled with name and date.
Standard #: 22VAC40-111-100-A Description: Based on record review, the family day home did not obtain documentation of a physical exam within 30 days of attendance.
Evidence:
1. Child # 1's first day of attendance was 9/26/19.
2. Documentation of a physical exam was not obtained by the provider within 30 days of attendance, and was not available for review.Plan of Correction: A copy of the physical exam will be requested from the parents.
Standard #: 22VAC40-111-60-B Description: Based on record reviews, children's records were not complete.
Evidence:
1. Child # 1's record was missing information for a second emergency contact.
2. An annual update for the records of Child #2, Child # 3 and Child # 4 was not completed annually.
3. Child # 5's record was missing the liability insurance form.Plan of Correction: Annual updates will be completed and missing information and paperwork will be collected.
