2. One of chile #7's bottles was not dated and one bottle did not have her name or the date.Plan of Correction: I will remind parents to label bottles and I will remind staff to label bottles if parents forget.
Standard #: 22VAC40-111-100-B Description: Based on a review of children's records, the family day home provider failed to obtain a physical examination for each child enrolled prior to attendance or by the 30th day of attendance.
Evidence:.
1. Child #5's record does not have documentation of a physical. The child's first day of attendance was 03/02/2017.Plan of Correction: I will ask the parent if they have a physical for the child.
Standard #: 22VAC40-111-60-A Description: Based on a review of children's records, the family day home provider failed to maintain required up to date records for children enrolled in the program.
Evidence:
1. Child #1's record did not have documentation of medical insurance.
2. Child #2's record did not have documentation of a physician's name address or phone number, names of persons other than the custodial parents who are authorized to pick up the child or a medical policy number.
3. Child #3's record did not have documentation of names of persons other than the custodial parents who are authorized to pick up the child or complete proof of identification.

Standard #: 22VAC40-111-210-A Description: Based on a review of annual training hours, the family day home provider failed to obtain and failed to ensure that caregivers obtain a minimum of 16 training hours annually.
Evidence:
Caregiver #1, #2 and #3 do not have documentation of the completion of 16 training hours for 2018-2019 year.Plan of Correction: We will complete training hours for 2019-2020 year.
Standard #: 22VAC40-665-240-C-2 Description: Based on observation, the family day home did not ensure that electrical outlets shall have protective covers that are of a
size that cannot be swallowed by children.
Evidence:
In the first nap room, one of the electrical outlets did not have a protective covePlan of Correction: I will replace outlet cover today.
