Evidence: Documentation was not maintained for emergency evacuation drills completed in the months of August 2019, September 2019, October 2019, and December 2019.Plan of Correction: Moving forward I will make sure to write down the dates of all fire drills.

Standard #: 22VAC40-111-180-A Description: Based on record review, 2 of 3 Adult Household Member files reviewed did not contain documentation of an updated tuberculosis screening or test completed within the last two years.
Evidence: Adult Household Member #2 and Adult Household Member #3's most recent tuberculosis tests were dated 12/29/2017.Plan of Correction: I will have them complete a tuberculosis test when they return home from school.
