Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted by the inspector and Licensing Administrator on February 4, 2020. The on-site inspection was from 10:20 a.m. to 1:10 p.m. On this date, there were eight children (fifteen points), ages two through five, in the care of the provider. Total enrollment is ten children. The exceptions to compliance were noted on the violation notice and the risks ratings were noted on the supplemental page. No medication is being administered, per provider?s policy. Program activities, daily routines, equipment, diapering and toileting procedures, posted information, chimney inspection, supervision, interactions with children, behavior management, caregiver/resident records, caregiver training, emergency supplies and drill records, pet record, five records for children, attendance record, menu, lunch preparation, and indoor and outdoor areas were reviewed. The children enjoyed a variety of toys and learning materials for use in different interest areas. There were opportunities for individual choices, exploration and creative expression as well as guided group discussions for learning experiences.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it within 5 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations:
Standard #: 22VAC40-111-240-A
Description: Based on observation, outside equipment was not maintained in a safe and operable condition.

Evidence: 1. The plastic green and white table.set contained a visible crack in one bench.

Plan of Correction: Provider immediately removed table from play area. Provider will check daily and make sure play equipment is in operable and safe condition.

Standard #: 22VAC40-111-680-B
Description: Based on observation, caregiver failed to ensure that children's hands are washed with liquid soap and warm running water before eating.

Evidence: 1. Children were observed sitting on floor watching television while lunch was being prepared. Once lunch was ready, children went to the table to eat without washing hands. 2. Caregiver confirmed that hands were washed prior to watching television, and was not aware that it needed to be done without other activities between.

Plan of Correction: Provider will make sure there is no delay between hand washing and eating snacks, breakfast or lunch. Hands will be washed immediately before sitting down at the table.
