Comments:

An unannounced renewal inspection was conducted February 27 2020 between 10:00am and 11:30am. There were 3 children in the direct care of the provider. Children were observed reading books, playing games, napping, and building. The home is clean and organized. Children had access to age-appropriate materials. Description:	Based on record review, 2 of 3 child records do not have documentation of all of the written information to be provided for parents. Evidence: Child #2 record does not have documentation that the parent or guardian received the Medication form, the emergency preparedness form, and the liability insurance form. Child #3 record does not have documentation that the parent or guardian received the medication form, and the liability insurance form.
Plan of Correction:	The parents will return them within two weeks.
Standard #:	22VAC40-111-90-A
Description:	Based on record review, 1 of 3 child records reviewed did not have documentation of immunizations prior to the child beginning care. Evidence: Child # 3 record does not have documentation of an immunization record.
Plan of Correction:	She will submit on March 16.
