Evidence: Child #1's record has one of two required emergency contact phone numbers. Child #3's record does not include the following: a signed parent acknowledgement form, proof of identity, a signed liability insurance disclosure and a signed medication policy.Plan of Correction: I will obtain all signed information from the parent.
Standard #: 22VAC40-111-80-A Description: Based on child record review, verification of the identity of one child was not obtained within 7 business days of the child's first day of attendance.
Evidence: Child #3 began care on 12/30/19. Verification of the child's identity has not been obtained.Plan of Correction: I will obtain the verification of identity this week, if parent''s are unable to provide the verification, I will be required to call law enforcement.
