Technical Assistance:

Information regarding risk can be obtained from the DSS website. Risk assessment discussed.
The current license and most recent inspection reports are to be posted in an area visible to the public.
As a mandated reporter all suspected abuse or neglect is required to be reported to CPS within 24 hours of suspicion.

The following FDH standards reviewed:
1. Standard 800. A - Written emergency plans - Model form for this can be obtained from DSS website;
2. Standard 320.A - Small electrical appliances shall be unplugged when not in use;
3. Standard 520.A - Cribs shall be provided for children from birth - 12 months;
4. Standard 60.A, 70.A and 80. A - Children's records, parent agreement and proof of age and identity for children;
5. Standard 570. A - points/need for additional caregiver;
6. Standard 680, 690 - Hand washing and diapering.

Comments:
An announced initial inspection was conducted on 03/20/2020 from 10:00 am - 11:40 am. At the time of entrance there were two children in care with the provider. The ages of the children counted for a total of eight points. Children were observed during morning program activity time. Lunch was served which consisted of a grilled cheese sandwich on wheat bread, sweet potatoes, grapes and milk.
Records were reviewed for the provider and two children in care. The entire home and backyard play area was reviewed.

The violations are listed on the violation notice and were reviewed with the provider at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-111-760-A-1
Description: Based on observation, the provider failed to ensure that the first aid kit shall contain all of the required elements.

Evidence:
1. The first aid kit lacked the following required items:
a. Two or more triangular bandages;
b. Antiseptic cleaning solution or pads.

Plan of Correction: These items will be obtained.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider failed to ensure that areas and furnishings of the family day home, inside and outside, shall be maintained in a safe condition.

Evidence:
1. The following toppling hazards were observed in the living room:
a. A flat screen television, measuring approximately 44 inches x 27 inches, has been placed on a entertainment table measuring approximately 50 inches in length.
b. These items have not been secured to the wall and do move when pushed or pulled on.
c. Children were observed in this room.
2. The following toppling hazards were observed in the child care room:
a. A box television, measuring approximately 16 x 9 inches, has been placed on a 3-shelf storage unit measuring approximately 47 x 3 inches.
b. These items do move when pulled on.
3. Four protruding screws were observed on the back of the play panel that has been attached the the outdoor backyard fence.
a. These screws have sharp points and are accessible if a child placed their hand/fingers behind the play panel which is mounted on the fence.
4. Two nails were observed protruding from under the top stair railing attached to the backyard stairway leading into the home.

Plan of Correction: 1. Both televisions and storage tables will be secured to the wall.

2. The nails behind the play panel will be covered.

3. The nails on the railing will be removed.

Standard #: 22VAC40-111-440-B
Description: Based on observation, the provider failed to ensure that protective barriers or guardrails shall be constructed to prevent a child from climbing over, crawling or falling through, or becoming entrapped.

Evidence:
1. There is an opening, measuring 21 inches, on the railing for the backyard stairs.
a. This opening is large enough to allow a child to fall through while walking up & down the stairs into the home from the backyard play area.

Plan of Correction: The missing spindle will be replaced.

Standard #: 22VAC40-111-690-E
Description: Based on observation and interview, the provider failed to ensure that soiled disposable diapers and wipes shall be disposed of in a leak-proof or plastic-lined storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal.

Evidence:
1. The trash can utilized for disposal of diapers was hand generated.
2. The provider stated that she did not have a step-down diaper pail or trash can.

Plan of Correction: A step down diaper pail or trash can with a step down feature will be obtained.
