This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on May 15, 2020 and concluded on May 15, 2020. The Provider and assistant were contacted by telephone to initiate the virtual inspection. There were 4 children present and 2 staff. The Inspector emailed the Provider a list of items required to complete the inspection and the Inspector reviewed children?s records shown by the facility to ensure documentation was complete. The staff and household records are the same and documentation verified past violations were corrected previously.
Information gathered during the inspection determined non-compliance with an applicable standards was documented on the violation notice issued to the facility or the program. Please call with questions 703-479-4679

Standard #: 22VAC40-111-60-B Description: Based on record review and interview it was determined one child's record did not contain required information.
Evidence:
1. Caregiver stated Child #4 started in care the beginning of May and is a sibling of Child # 3.
2. The record contained an agreement but did not contain if the child had any allergies or health concerns.Plan of Correction: Parent has the form to list all information and we will have it next week.
