This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 05/21/2020 and concluded on 05/22/2020. The provider was contacted by telephone to initiate the inspection. There were 6 children present with the provider. The ages of the children counted for a total of 12 points. The inspector emailed the provider a list of items required to complete the inspection. The inspector reviewed 2 children's records and 2 staff records submitted by the provider to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the provider.
The licensing inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.

Standard #: 22VAC40-111-100-A Description: Based on record review, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Documentation of a current physical exam was not obtained within 30 days of enrollment as required for child 1 who has an enrollment date of 06/03/2019.
a. The physical provided for child 1 was obtained on 05/28/2020.Plan of Correction: Documentation of a current physical exam will be obtained within 30 days of enrollment going forward.
