Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. An initial inspection was initiated and concluded on June 15th, 2020. The provider was contacted by telephone to initiate the inspection. There was 1 child present and 1 staff. The Inspector reviewed 4 children's records and caregiver and household member records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Standard #:	22VAC40-111-60-B
Description:	Based on a review, 4 of 4 children's records reviewed did not have all required documentation. Evidence: 1. Child #1's record did not contain documentation of the doctor's address or names of persons other than the custodial parents who are authorized to pick up the child. 2. Child #2, #3 and #4's records did not contain documentation of the name of persons other than the custodial parents who are authorized to pick up the child.
Plan of Correction:	I will get the information from the parents.
Standard #:	22VAC40-111-70-A
Description:	Based on a review, 4 of 4 children's files reviewed did not have all of the required documentation within each child's file. Evidence: 1. Child #1, #2 ,#3 and #4 did not have a signed Policy for the Administration of Medication for the Family Day Home. 2. Child #1, #2, #3 and #4 did not have a signed review of the Family Day Home's Liability Insurance Declaration form based on 63.2-1809.1 of the Code of Virginia.
Plan of Correction:	Missing documentation will be completed and signed and placed in children's records.
