Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An announced initial inspection was initiated and concluded on 07/22/2020. The provider was contacted by telephone to initiate the inspection. There were four children present and one staff. Several standards could not be determined today because the facility is not fully operational. The Inspector reviewed one staff record and one household member?s record to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-330-A
Description: Based on interview, the provider failed to have an operable landline telephone.
Evidence:
1. The inspector asked the provider if the home has an operable landline telephone. The provider said the landline telephone has been disconnected.

Plan of Correction: The provider will contact the telephone company and request the landline telephone be reconnected. The provider will inform the licensing inspector when the landline telephone is operable.
