Standard #: 22VAC40-111-60-B Description: Based on record review, the provider did not obtain a physical examination by or under the direction of a physician within 30 days of attendance for a child in care.
Evidence:
Child#1's record does not contain a physical examination. The child's first day of attendance was documented as 9/30/2019.Plan of Correction: Necessary form is given to the baby's mom. She have doctor appointment and will bring back soon. [SIC]
