Standard #: 22VAC40-111-100-A Description: Based on record review, the provider did not obtain documentation of a physical examination for a child within 30 days after the first day of attendance.
Evidence:
1. Child # 1's first date of enrollment was 7/29/19.
2. Child # 1's record had a form used for a physical exam but it was not completed by the child's physician. It was an incomplete physical exam form.Plan of Correction: I will ask the parent for the form.
Standard #: 22VAC40-111-90-C Description: Based on record review, the provider did not obtain additional immunizations for a child who is not exempt from the immunization requirements.
Evidence: Child # 1's (18 months of age) record had immunizations that were last updated in April 2019. Additional updated immunization records were not obtained after that date.Plan of Correction: I will ask the parents to bring me updated immunization records.
