Technical assistance was offered regarding children's health forms.

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A (monitoring/renewal) inspection was initiated and concluded on 8/4/20. The provider was contacted by telephone to initiate the inspection. There were 4 children present and 2 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Standard #: 22VAC40-111-100-A Description: Based on record review, the provider did not obtain documentation of a physical examination for a child within 30 days after the first day of attendance.
Evidence:
1. Child # 1's first date of enrollment was 7/29/19.
2. Child # 1's record had a form used for a physical exam but it was not completed by the child's physician. It was an incomplete physical exam form.Plan of Correction: I will ask the parent for the form.
Standard #: 22VAC40-111-90-C Description: Based on record review, the provider did not obtain additional immunizations for a child who is not exempt from the immunization requirements.
Evidence: Child # 1's (18 months of age) record had immunizations that were last updated in April 2019. Additional updated immunization records were not obtained after that date.Plan of Correction: I will ask the parents to bring me updated immunization records.
