Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 8/05/2020 and concluded on 8/05/2020. The director/provider or in-charge person was contacted by telephone to initiate the inspection. There were 6 children present and 1 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff/household records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review, the Licensee failed to obtain documentation of a physical examination by or under the direction of a physician prior to attendance or within 30 days after the first day of attendance.

EVIDENCE:
Licensing Inspector reviewed 2 children records. Record for Child #2 did not include documentation of the child's physical examination. Child #2 first date of attendance was on 2/28/2020

Plan of Correction: Provider will obtain documentation of the child's physical examination from the parents. Upon receipt, documentation will be maintained in the child's record.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record review, the Licensee failed to complete a search of central registry before five years since the date of the most recent central registry finding.

EVIDENCE:
Licensing Inspector reviewed 2 staff-household records. Record for Staff/household member #2 did not include documentation of an updated central registry background check. The most recent central registry background check for staff-household #2 is 2/12/2015.

Plan of Correction: Provider will submit documents to update central registry background check. The provider will send documentation to the Licensing Inspector that the background check has been requested no later than 10 days following this notification date (August 13, 2020)
