Technical Assistance:
Provided consultation on:
1) Standard 560: Monitoring each sleeping infant by: a) placing the infant to sleep in a location within sight and sound supervision; b) by in-person observation of each sleeping infant at least once every 15 minutes; or by c) using a baby monitor
2) Standard 580: Providing opportunities for vigorous outdoor play daily
3) Standard 630: Caregivers shall use positive methods of discipline. Discipline shall be constructive in nature. When time out is used, it shall be used sparingly and shall not exceed one minute for each year of the child?s age; time out shall not be used with infants or toddlers (0-24 months.)
4) Standard 870: Scheduling snacks or meals so there is a period of no more than three hours between each meal or snack.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 8/27/2020 and concluded on 8/28/2020. The provider was contacted by telephone and a virtual inspection was conducted. There were five children present, ages 16 months - six years of age, for a total of nine points. The Inspector reviewed three children's records, five caregiver/household member records, the menu, emergency drill log, pet vaccinations, hand washing and diapering procedures, enhanced cleaning and sanitizing of toys and surfaces, and daily indoor and outdoor activities. The information gathered during the inspection determined non-compliance with standards that are documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-111-170-A
Description: Based on review of five caregiver/household member records, and an interview with the provider, not all adult household members had an initial TB test/screening. Evidence: the record for household member #3, (date of birth: 12/27/2001) did not contain documentation of a negative TB test/screening.

Plan of Correction: Will schedule an appointment for a TB test/screening.

Standard #: 22VAC40-191-40-D-4-a
Description: Based on review of five caregiver/household member records, and an interview with the provider, it was determined that not all household members had a sworn statement when they turned 18 or Central Registry Search results within thirty days of turning 18. Evidence: The record for household member #3 (date of birth: 12/27/2001) did not contain a sworn statement or documentation of a Central Registry search report.

Plan of Correction: A sworn statement was completed during the inspection. Provider stated a Central Registry search was conducted, and she will locate the documentation or resubmit a request.

Standard #: 63.2-1721.1-B-2
Description: Based on review of five caregiver/household member records, and an interview with the provider, not all household members had an Office of Background Investigations (OBI) fingerprinting-based criminal history record determination letter at the time they turned 18 years old. Evidence: the record for household member #3 (date of birth: 12/27/2001) had a fingerprint determination letter dated: 8/26/2020.

Plan of Correction: A fingerprint determination letter is now in the record.
