Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An unannounced focused monitoring inspection was conducted on 9/15/20 as a follow-up to a previous complaint inspection. The provider was contacted by telephone to initiate the inspection. The Licensing Inspector interviewed one staff member to determine compliance with applicable standards and law. Information gathered during the inspection determined non-compliance and a violation was documented on the violation notice issued to the facility.

Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice and return to me by 5:00pm on 9/30/20. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 63.2-1721.1-C
Description: Based on review of household and provider record and an interview with the provider, it was determined that a household member has a disqualifying central registry finding. Evidence: Household member A has a founded disposition of child physical abuse. Household member A is also an assistant in the licensed family day home.

Plan of Correction: Household member A will be moving out and finding another job by October 12, 2020.
