Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on January 4, 2021 and concluded on January 5, 2021. The provider was contacted by telephone to initiate the inspection. There were eight children present and two staff. The inspector emailed the provider a list of items required to complete the inspection. The inspector reviewed two children?s records and one staff record submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.

Standard #:	22VAC40-111-60-B
Description:	Based on review and interview, provider did not ensure 2 of 2 children's records had documentation of required information. Evidence: The following was found during record review: 1. The record for child #1 (start date June 1, 2020) did not have complete addresses for parent's place of employment and the address for the second emergency contact. 2. The record for child #2 (start date October 11, 2020) did not have father's information and a second emergency contact. 3. Missing information was verified by the provider.
Plan of Correction:	Provider contacted parents during the inspection and got missing information. Information was added to the records.
Standard #:	22VAC40-111-200-A
Description:	Based on review and interview, provider did not ensure assistant or substitute receives orientation training by the end of their first week of assuming job responsibilities. Evidence: The record for staff #1 (start date of employment November 8, 2020) had documentation that the orientation training was completed November 30, 2020. Information was verified by the provider.
Plan of Correction:	In the future, provider will ensure orientation training is completed within the first week of beginning employment.

Description:	Based on review and interview, provider did not ensure a employee has a completed sworn statement or affirmation prior to beginning employment. Evidence: The record for staff #1 (start date of employment November 8, 2020) had a sworn statement or affirmation signed and dated on November 30, 2020. Information was verified by the provider.
Plan of Correction:	In the future, provider will ensure sworn statement or affirmation is completed by any new employee prior to beginning employment.
