Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was conducted on 3/10/2021. The provider was contacted by telephone to initiate the inspection. There were 12 children present and 2 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records, 3 staff records, and one household member record submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 4:00pm on Wednesday 3/17/21. Standard #:	22VAC40-111-100-A
Description:	Based on record review, the provider did not obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. Evidence: Child #3 (First day of attendance: 9/13/2020) did not have a physical examination on file.
Plan of Correction:	Asked for family to provide by 3/17/2021 or child will be removed.
Standard #:	22VAC40-111-60-B
Description:	Based on record review, the provider did not ensure that children's records contained all required information. Evidence: Child #1, Child #2 and Child #3 were missing name and policy number of the child's medical insurance. In addition, Child #3 was missing addresses for 2 of 2 required emergency contacts.
Plan of Correction:	Adding address to emergency record. Will obtain medical cards to be added to child files.
Standard #:	22VAC40-111-80-A
Description:	Based on record review and provider interview, the provider did not obtain verification of identity and age within seven business days of children's first day of attendance at the family day home. Evidence: Child #1 (First day of attendance: 2/17/21) and Child #3 (First day of attendance: 9/13/20) did not have documentation that proof of identity and age was verified.
Plan of Correction:	Asked for proof via message. Giving until 3/12.
Standard #:	22VAC40-111-90-A
Description:	Based on record review, the provider did not obtain documentation that all children have been adequately immunized before attending the family day home. Evidence: Child #3 (First day of attendance: 9/13/20) did not have the immunization record on file.
