Comments:
A Subsidy Health and Safety inspection was conducted on 05/30/2019 from 11:30 a.m. to 1:45 p.m. to review supplemental health and safety requirements. There was 1 child present, 23 months old for a total of 3 points, in care with the provider. The records sample size consisted of six enrolled children?s records and 1 vendor record. The child was observed playing with toys, talking and snuggling with the provider, reading books and resting on a cot. The vendor states that no medication is administered to children in care at this time and that children may be transported in a vehicle. Requirements for transportation were reviewed with the vendor - but transportation was not observed during the inspection. Children play outdoors at a nearby neighborhood park with direct supervision and close proximity of the provider. The provider reports that there have been no accidents or injuries requiring first aid or medical treatment incurred by children in care since the last inspection. Thank you for your time during today's inspection.

Violations:
Standard #: 22VAC40-665-170-A
Description: Based on a review of records, the vendor did not maintain a record containing all of the required components. Evidence: The records for child #1, #2, and ## (siblings) do not contain the addresses for the 2 emergency contacts, designated by the parent to contact in case of an emergency if the parent cannot be reached.

Plan of Correction: The vendor stated that she will obtain the addresses for the emergency contacts by the end of the week.

Standard #: 63.2-1721.1-B-2
Description: Based on a review of records, the vendor has not submitted to fingerprinting for the purpose of obtaining a federal criminal record search. Evidence: The vendor's record did not contain the results of a federal criminal records search. The vendor stated that she had not been fingerprinted because she was unsure of how to go about obtaining them. The vendor's record did have a State Police background check.

Plan of Correction: The vendor was given instructions for obtaining fingerprinting during the inspection. The vendor stated that she would make an appointment for fingerprinting by the end of the week.
