This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on May 28, 2021 and concluded on May 28, 2021. The provider was contacted by telephone to initiate the inspection. There were six children present and two caregivers. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.
The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-665-230-H Description: Based on review of caregivers records, the provider did not ensure caregivers completed the annual Virginia health and safety Updated training for child care staff required annually of subsidy vendors and their staff.
Evidence:
1. The most recent completion of the health and safety annual training for caregiver #1 was dated 04/01/2019. Caregiver #1 did not have documentation of completing the annual health and safety training 12 months from the original date of completion for 2020 and 2021.
2. The most recent completion of the health and safety annual training for caregiver #2 was dated 03/29/2019. Caregiver #2 did not have documentation of completing the annual health and safety training 12 months from the original date of completion for 2020 and 2021.Plan of Correction: Per the provider: We will complete the health and safety updated training as soon as possible and every year there after.
